Disclaimer: Eligibility and Verification of benefits is required before initial visit. Most PPO plans with out of network coverage are accepted for qualifying conditions. Not all conditions, treatment modalities and services are covered by insurance. Insurance benefits are limited to office visits for acupuncture and are limited to specific symptoms and conditions as deemed medically necessary by your insurer. Please check with your insurer for what conditions they cover or reach out with any questions. Verification may take 1-2 business days for some plans. Kaiser patients may need a referral on file before an initial visit.

Out of Network Insurance
Out of Network coverage is available on PPO plans for several major carriers. In most cases, a copayment (or coinsurance payment) is required after the deductible is met and visits can be billed and applied towards the deductible. For plans without acupuncture or out of network benefits superbills are provided on request.
FSA/HSA plans are accepted.
A card on file is required for out of network billing unless paid at time of service.
A limited number of personal injury liens and Medway cases are seen per year.
